I am integrating the video egg publisher into my website and I need a customized form and custom script prepared to complete the upload movie action. The details of the publisher can be found within the attached document. (please review before responding) Step 6 of the documentation refers to embeding the publisher as a form element. I need a custom form created that will allow me to capture user provided metadata, like name, title and description that also has the video egg publisher embedded as a form element.
