Nutrition Distiller - new application I need to have an internet application developed and installed. I believe the scope is fairly well defined but I am very open as to exactly how the necessary requirements are implemented. Simply the application consists of the following main components: #Administration of 3 user types #Questionnaire with reporting #Minimal Information processing #Paypal interface (single product) Simply, a questionnaire is filled out; the results are processed and result in a report. Clients must pay to access the report. The 3 user types are: Admin, Associate and Client. Basically it’s similar to lots of survey software but with a few extensions and customization. I intend to host the application at Godaddy.com on Linux using PHP/mySQL. As the application is new it can be tested where it will go live. I will be conducting the user acceptance testing. The web design has not been done so all I am after is functionality right now. Eventually the application will have multiple web front ends appealing to different markets, so modularity is important. I require a service provider who is communicative and will make the effort to understand what is required. This project is the start of what could become an extensive development. A job well done will be rewarded with more work and bonuses for impressive results and constructive advice. Please provide samples of previous custom work. The requirements include the design and setup of the mySQL database. Newly added descriptions: I only require PHP and mySQL setup with enough HTML the show functionality. No design etc. is required. |
