I have a domain that is hosted by an ISP. The ISP has provided this tutorial on installing a form processing script here: http://www.lunarforums.com/forum/index.php?topic=12593.0 I need to setup this and try to convert the attached 2 page documnet (2 pages represent 1 document) and there is another one document like that, into an online form. the idea is for the customers to fill it in and to email it before the visit. Staff would need to be able to print it in some readable format, or even better to be able to print it as it was filled in on the screen. I need someone that has done this before, and that can be online for longer periods of time so we can finish the job ASAP. I may be able to organise a FTP access tot he site. Please reply to this with some of your ideas on how to do it.. I do not see this being a big job as we are only talking about two forms. The site uses TFMail script for sending the emails.
