| Project Description:
I need a php page edited so that each of the options have the correct input fields as describe by the requirements below. The user will be selecting one of the options. The options they select should be saved in a database (one table that you create/design) so when the user accesses this page again, they see the options that they selected previously. When the user clicks on "generate code" for each option, the form code needed is generated so they can copy/paste it onto their own web page. Attached is the correct form code for "generic" - you will need to create the code for the other options based on the particular payment method given i the requirements below. The requirements for each option are attached in the zip file along with the PHP file to be edited. If you complete this job properly, I will have a follow up job which involves integrating the generated code with the order placement code.
