| Project Description:
I am conducting a telephone survey and want to be able to enter the data into excel using a form. I want the user to be able to open the excel survey workbook and click a ??tart survey??button to launch the user form. The form must begin with a drop down box for the person conducting the survey to select their name. This must be followed by a set of text boxes for the respondent?? name, IFA, policy number and policyholder. The user form must then list the 25 or so questions with buttons to select a level of satisfaction between 1 and 10, or N/A (not applicable). In addition approximately every fifth question there is the option for the respondent to give additional information which I want to be able to record in text boxes on the user form. Once the survey has been completed I want to be able to click a ??inish??button on the user form and then have all of the information entered from the user form into the first empty row on the excel worksheet.
