I need minor fixes to two forms that are already created and captcha security image function installed on one. These forms do NOT need to be connected to a database as information is sent direct to my email address. The forms and their attendant scripts are attached in a zip file. 1) One ( http://editfast.com/english/writing_contests/essay_writing_contests/essay_writing_contests.htm ) is simply to accept the agreement/rules before registering, but a). the confirmation needs to be connected to the user info (which is input on the next page) so I have a record of who has accepted the agreement. b). The check box is REQUIRED 2) ( http://editfast.com/english/writing_contests/essay_writing_contests/essay_writing_submissions.htm ) Is to: a). get the user info b). upload the user's document c). send the info and document directly to me by email. d). Document types must be limited to .doc, .docx, .pdf, .txt, .wps, .wpd etc. (all major word processing formats) and .zip e) All fields are REQUIRED (including the upload field.) f). This form should not be accessible to anyone except those who have accepted the agreement via form one above. |
